Resident-Centered Care Be part of a team committed to helping residents recover, regain independence, and achieve their highest level of function. Your Impact & Responsibilities Clinical Oversight Provide and oversee nursing care for a designated group of residents. Ensure medications and treatments are administered accurately and safely. Acute Assessment Conduct comprehensive nursing assessments. Monitor residents for changes in condition and communicate findings promptly to physicians and the interdisciplinary team. Care Coordination Participate in admissions, discharges, and care planning activities. Collaborate with residents, families, and healthcare team members to ensure continuity of care. Team Support Provide guidance and support to LVNs and CNAs. Promote compliance with facility policies, procedures, and clinical standards. Documentation Maintain accurate and timely electronic medical records in accordance with state and federal regulations. Qualifications Current and active California Registered Nurse (RN) license in good standing. Previous experience in a skilled nursing, post-acute, rehabilitation, or sub-acute setting preferred. Strong clinical assessment, communication, and organizational skills. Ability to thrive in a fast-paced, team-oriented environment.
