Legacy Hospice and Palliative Care, located in Chino, CA, is currently looking for compassionate and caring per-diem RN's, LVN's and CHHA's to care for our patients in the Central and Southern Orange County, Irvine, Santa Ana, Costa Mesa, Newport, Laguna Woods and Lake Forest and its surrounding areas, (Laguna Woods and Lake Forest is the furthest area of coverage of southern Orange County) Candidates must reside locally to the Central and Southern Orange County area. RN:

Per-diem rate can range from $85.00 to $350 (per RN visit) and is dependent on the type of visit being provided.

LVN:

Per-diem rate can range from $40.00 to $55.00 (per LVN visit) and is dependent on the type of visit being provided

CHHA:

Per-diem rate can range from $35.00 to 40.00 per visit Duties Perform comprehensive patient assessments, vital signs, and physical examinations to determine individual needs and develop tailored care plans. Administer medications via injections, IV infusions, and other methods following strict aseptic techniques and medication protocols. Document all patient interactions accurately using EMR (Electronic Medical Records) systems like HospiceMD while ensuring compliance with HIPAA regulations. Collaborate with multidisciplinary teams on discharge planning, case management, and triage to optimize patient recovery and safety. Support patients across the lifespan including geriatrics, memory care residents, and individuals with developmental disabilities or dementia. Qualifications Valid licensure as an RN (Registered Nurse), LVN (Licensed Vocational Nurse), or CHHA (Certified Home Health Aide) with current certification in good standing. Experience working in hospice care is a must. Proficiency in EMR/EHR system HospiceMD. Strong knowledge of medical terminology, anatomy, physiology, and diagnostic evaluation techniques. Excellent communication skills for effective patient assessment and health coaching; ability to work independently and within a team environment.
