We are growing, and we are looking for a Director of Nursing who is compassionate, organized, proactive, and genuinely enjoys spending time with fellow nurses and older adults. Nursing is about more than checking boxes. Our founder, who is also a Registered Nurse, has built this from scratch and we want to add another dynamo to this mix. What You'll Do As our Director, you will serve as a trusted clinical resource and advocate for our clients and their families. You will also support other team members when they have clinical questions. Your time will be divided between maintaining a client caseload of up to 25-30 billable hours per week and the remainder of your time will be dedicated to overseeing and supporting our Nurse Care Managers and Concierge Nurses, ensuring quality of care, consistency, communication, and an exceptional client experience. You will be responsible for overseeing, maintaining and improving our Nurse Care Management program and Concierge Nursing services. This is why we need a nurse who is a true leader. Client Care & Clinical Oversight Conduct comprehensive assessments of older adults and their care needs Develop individualized care plans based on each client's goals, needs, and preferences Monitor changes in clients' physical, cognitive, emotional, and social needs Provide medication oversight and help families navigate complex medical situations Attend physician appointments and help clients and families understand recommendations Communicate with physicians, therapists, facilities, pharmacies, caregivers, and other members of the care team Coordinate services and resources to ensure clients receive the right care at the right time Make recommendations regarding level of care, safety, and additional services Provide nursing services within your scope of practice when needed Complete home visits and follow up proactively with clients and families Be a cheerleader to the members of our team which include caregivers and Registered Nurses Be the Person Who Sees What's Coming This role is about more than responding to problems. We need a nurse who anticipates needs, connects the dots, follows through, and takes ownership. You may notice that a client is becoming less steady on their feet, recognize that medications aren't being managed well, realize a family caregiver is overwhelmed, or see that a client's current care plan isn't working — and then do something about it. Comfortable communicating with physicians, families, caregivers, and other professionals Someone who genuinely enjoys getting to know their clients (our business is all about relationships…we love the people we serve) A team player who contributes to a positive and supportive workplace Comfortable with technology and electronic documentation (We can train on our systems but we want someone who can navigate a computer and pick up technology quickly. You will be working on a desktop but also through apps) Most importantly, we want someone with a big heart who truly enjoys caring for people. Schedule & Work Environment Full-Time | Monday-Friday (mostly….this is an exempt position and you are expected to available afterhours and weekends) This position is a combination of: Working from our Palm Desert office Visiting clients throughout the Coachella Valley Attending medical appointments and care meetings Completing some administrative/documentation work remotely as appropriate Compensation $115,000/year to start. What You Bring Required:

Active California Registered Nurse (RN) license Experience working with older adults Excellent verbal and written communication skills Strong organization and time-management skills Ability to prioritize and manage multiple clients, teams and responsibilities Ability to work independently in both an office and field environment Reliable transportation and valid driver's license Current CPR certification Comfortable using technology and electronic documentation systems A willingness to learn (so important!) A plus: Experience in geriatrics, home health, hospice, critical care, or care management Experience coordinating care for medically complex clients Knowledge of senior resources and services in the Coachella Valley Why Heart of Gold?
