Monday- Friday with on call weekends rotation As a Home Health RN, you will: Provide skilled, compassionate nursing care in accordance with the nursing process, physician orders, and the patient's individualized Plan of Care (POC). Assess and monitor patient condition, regularly re-evaluating needs. Administer medications, treatments, and specialized procedures within scope of practice, training, and demonstrated competency. Communicate changes in patient condition and care needs with physicians, supervisors, patients, and families/caregivers to promote continuity of care. Educate patients and families/caregivers on medications, treatment plans, disease management, safety, and self-care to support independence. Complete accurate, timely, and thorough clinical documentation in accordance with Lorian policies, procedures, and regulatory requirements. Promote a safe, efficient care environment while supporting activities of daily living, appropriately delegating/supervising staff, and maintaining professional knowledge through ongoing education. Required Qualifications Is a graduate of an accredited school of professional nursing, BSN preferred. Currently licensed to practice as a RN in the state of practice. If IV or ET cer tified, is currently certified and recognized by the state(s) where currently practicing as an RN. Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting. Possess and maintains a valid driver's license. This position is based on a pay per visit compensation model. Pay per visit ranges from $100 -180 visit commensurate with experience and depending on type of visit.
