In this role, you will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. You will collaborate with a multidisciplinary team to develop and implement individualized care plans while promoting patient independence and quality of life. Duties Conduct comprehensive assessments of patients' health status and needs in their home environment. Administer medications and treatments as prescribed, including IV insertion and catheter care. Utilize electronic health record systems such as eClinicalWorks and Epic to document patient information accurately. Educate patients and their families on health management, medication administration, and disease prevention. Collaborate with physicians, nursing homes, and other healthcare professionals to coordinate patient care. Monitor patients for changes in condition and respond appropriately to emergencies or complications. Stay updated on ICD-9 and ICD-10 coding for accurate documentation of diagnoses and procedures. Provide support in trauma medicine situations when necessary. Experience Candidates should possess the following qualifications: Current Registered Nurse (RN) license in the state of practice. Previous experience in home health care or a teaching hospital setting is preferred. Familiarity with electronic health record systems such as eClinicalWorks or Epic is a plus. Strong clinical skills, particularly in IV insertion and catheter care. Excellent communication skills for effective patient education and collaboration with healthcare teams. Ability to work independently while managing multiple patient cases efficiently.
