Job Description

Home Health LVN or Registered Nurse (RN) We are seeking a compassionate, dependable, and skilled Home Health Registered Nurse (RN) or Licensed Vocational Nurse (LVN) to join our growing care team. The ideal candidate is a team player who can adapt quickly in a fast-paced environment and take full responsibility for patient visits, clinical care, and accurate documentation.
RATE:
RN $170-180 SOC and recert, $80 for follow up visit, for LVN visit $65, PT is $110 per visit, OT is $105
Location:
Around Frazier Park Area Responsibilities Conduct scheduled home health visits Perform patient assessments and vital signs Provide skilled nursing care within LVN scope Complete accurate and timely EMR/EHR documentation Educate patients and caregivers Communicate with RNs, physicians, and care team Qualifications Active LVN/LPN license Must have an experience on using EHR DSL for documentation (non-negotiable) Home health or clinical experience preferred EMR/EHR proficiency Strong documentation and time-management skills Reliable transportation Compensation (Per Visit) $50 - $60 - Follow up visit For Compensation Purposes Join a supportive home health team and make a meaningful impact through patient-centered care.
