This position is responsible for providing skilled nursing care and services to patients in their place of residence, and may case manage nursing and non-skilled care and services when assigned. All nursing services are performed in compliance with state and local laws/regulations, the Nurse Practice Act, and all other healthcare accreditation standards.

DOE What You'll Do:

Initiate and identify patient needs through assessment Provide nursing services based on the plan of care Prepare necessary items for treatments and procedures Implement and follow current standards to prevent the spread of infection; instruct patients on and, as appropriate, prepare medications for their care and safety Make treatment recommendations based on the patient's acuity level and goal potential Excel at the admissions process, including same-day electronic documentation Remain flexible to accommodate admissions within time, travel, or geographic constraints Provide and document skilled care on all visits, meeting agency documentation standards Drive throughout the day to visit patients in their homes

Requirements:

Valid California Registered Nurse license A minimum of 2 years of experience in a Home Health or Acute Care nursing setting Current CPR certification Valid California Driver's License Computer skills are a must, including typing skills for reporting and updating documentation in an online portal Willingness and ability to work with electronic input and telecommunication devices Excellent clinical skills and demonstrated ability to manage time well Excellent communication skills; able to communicate clearly with staff and the general public, both verbally and in writing All applicants/contractors are considered without regard to race, color, ethnicity or national origin, religion, gender, age, marital status or sexual preference, veteran status, physical or mental handicap, or any other legally protected status.
