13 weeks contract with possible extension Pay rate: $65 - $75/hr Job Overview We are seeking a dedicated and experienced Registered Nurse (RN) Case Manager to coordinate and manage patient care across the healthcare continuum. The RN Case Manager will work closely with patients, families, physicians, nurses, social workers, insurance providers, and other healthcare professionals to develop and implement individualized care plans, promote positive patient outcomes, and ensure appropriate utilization of healthcare resources. The ideal candidate will have strong clinical judgment, excellent communication and organizational skills, and the ability to effectively coordinate care in a fast-paced healthcare environment. RN Case Manager Responsibilities Assess patients' medical, functional, psychosocial, and discharge planning needs. Develop, implement, monitor, and evaluate individualized patient care plans. Coordinate care and services among physicians, nurses, social workers, therapists, patients, and other healthcare providers. Facilitate safe and timely patient transitions between hospital, home, rehabilitation, skilled nursing, and other healthcare settings. Collaborate with the healthcare team to identify barriers to care and develop appropriate solutions. Monitor patient progress and ensure care plans are updated based on changing clinical needs. Coordinate referrals, follow-up appointments, home health services, durable medical equipment, and other post-acute services. Assist with utilization management and ensure appropriate use of healthcare resources. Communicate with insurance companies and other payers regarding authorizations, continued stay, and discharge needs. Educate patients and families regarding treatment plans, discharge instructions, available resources, and ongoing care needs. Maintain accurate and timely documentation in Electronic Medical Records (EMR/EHR) systems. Ensure compliance with hospital policies, regulatory requirements, and established standards of care. Advocate for patients and support continuity of care throughout the healthcare journey. Requirements RN License BLS Case Management Certificate.Pay:
