Job Summary:
The Primary Care Nurse's primary role is to provide support to providers seeing patients in the clinic by doing nurse start visits, flip visits, medication reconciliation, Transition Care, administering medications that cannot be administered by medical assistants, wound care, provide patient education and more. Work is centered around the needs of patients being seen in the clinic that day. As capacity permits, the Primary Care Nurse may need to cover for other RN positions as well as other sites during staffing shortages. Position is posted for 40 hours/week, Mon-Fri, 8:45am-5:15pm and is located fully on-site at our Russian River Health & Wellness Center in Guerneville, California. New grads welcome to apply!
Responsibilities:
View the providers' schedules ahead of time, identify patient visits likely to need RN input, and plan the Primary Care Nurse schedule. Communicate with providers and care team members accordingly. On the day of the visits, manage, or plan to manage any additional needs in and around the day's patient visits. Perform medication reconciliation for patients with polypharmacy and/or with multiple chronic conditions and recent medication changes. Complete nurse start visits and flip visits that may include low to medium risk Transition Care, ED follow up, hypertension management, controlled medication refills, STI checks, UTI, wound care and other types of visits needing support from a nurse. Administer medications as ordered by provider, including Rocephin, insulin, B12, testosterone, puberty blockers and others as requested. Provide patient education. Start and manage IV fluids infusions. Administer assessments that may include MoCA or SLUMS. Assist with patient care items which may include medication refills per written RN guidelines. Communicate with colleagues within West County Health Centers, and with outside healthcare organizations, including hospitals and home health agencies. Provide back up to fellow Clinic Nurse as needed, including telephone triage. Assist with patient transfers to ED. Oversee Care Team Medical Assistant administration of immunizations and medications, both injection and oral within the office visit. Work with Care Team to actively manage the patient schedule to optimize efficiency, patient experience, and meet the needs of the patient. Assist in the coordination of warm hand-offs or "Transfer of Trust" between Care Team, Behavioral Health Staff, or other support services during the office visit.
MINIMUM REQUIREMENTS
Current California Registered Nurse License Completed board-approved associate (ADN) or bachelor (BSN) nursing program Current CPR certification
EXPECTATIONS
Ability to work well on a multidisciplinary team. Ability to problem solve and be self-motivated with acute attention to detail. Strong organizational skills and the ability to multitask effectively in a busy environment. Expected to have strong interpersonal skills and can understand and follow written/verbal instructions. Ability to work independently with little or no supervision. Excellent interpersonal skills to interact positively with patients and staff alike. Proficiency in using office software and electronic health record systems is beneficial.
PHYSICAL REQUIREMENTS
Ability to bend and move arms with sufficient range of motion to complete patient related requirements, hearing required for patient assessments, may need to assist patients onto exam table, able to lift 30 lbs., bend, kneel and twist as needed. Prolonged standing and occasional sitting required. Must be able to perform the essential duties with or without accommodations. Basic office and equipment safety knowledge and practice.
