  • $85 an hour 2 days ago Benefits Flexible schedule Qualifications CPR Certification RN License BLS Certification Mobility assistance Initial patient assessment Meal preparation Critical care experience Full Job Description TLC Nurse Solutions is seeking experienced independent contractor RNs for an ongoing private-duty concierge nursing opportunity in Hanford, CA , beginning approximately late September/early October 2026 .
The patient is recovering from a recent lung transplant and will be transitioning home after receiving private-duty nursing support.
Schedule:
12-hour shifts, primarily 7 AM-7
PM Position:
Independent Contractor (1099) Nursing Care Includes Post-lung transplant assessment and monitoring Medication management and blood glucose monitoring Infection-prevention precautions Mobility assistance and fall prevention Hands-on assistance with ADLs, including toileting and personal care Meal preparation and nutrition/hydration support Daily weight, vital signs, and ordered assessments Detailed nursing documentation and communication/handoff with the care team Collaboration with physicians, transplant coordinators, specialty pharmacies, and other members of the healthcare team to promote continuity of care This is a hands-on concierge nursing case . In addition to skilled nursing care, the RN should be proactive, attentive, and comfortable anticipating and assisting with the patient's day-to-day needs. Qualifications Current California Registered Nurse (RN) license in good standing Minimum of two years of recent RN experience ICU, PCU, Step-Down, Pulmonary, Cardiac, Transplant, Acute Care, or high-acuity home care experience preferred Experience caring for medically complex patients in the home or acute care setting preferred Experience with transplant patients, or other high-acuity clinical care is a plus IVIG and/or home infusion experience is a plus and may provide opportunities for additional assignments through TLC Nurse Solutions Excellent assessment, communication, and critical-thinking skills Current BLS certification required; ACLS preferred We are looking for dependable, attentive RNs interested in becoming part of an ongoing nursing team and providing consistent, coordinated care.
