Duties Include:
The registered nurse is expected provide direction to members of the patient care team through case management of a patient. Case management of patient services includes: Perform initial evaluation, follow up visit and discharge of your patient Design a care plan, and medical record baseline, and begin implementation of that care plan Communicate with other members of patient care team and adjust care plan as needed Reach out for community services to assist patient as needed Physical treatments with patients as per designed care plans Review notes/charts of LVNs and sign off on their notes, communicating with them regularly. Appropriate supervision of patients that have been handed to LVNs Collaborate with physicians, other agency staff and contract services to coordinate and implement the Plan of Care Trains new clinicians by allowing clinicians to shadow patient visits Documents direct patient services, coordination and collaboration with physician and other disciplines Promotes the agency and services to the public and participate in physician and community education related to services provided by the agency as registered nurse. Appropriate discharge planning Benefits Visits are available up to $130- $150/visit for Start of Care .
