Job Description

Job description We are seeking an LVN Field Nurse to join our team to visit home health patients in Orange County and surrounding areas. The successful candidate will be responsible for providing home health or hospice care services. The Field Nurse must be able to work independently and have a valid driver's license.
Responsibilities:
Provide home health or hospice services to patients in their homes. Provide patient education and support. Monitor patient progress and modify plans of care as needed. Document patient care in accordance with agency policy and procedures.
Qualifications:
Currently licensed vocational nurse in the state of California. Reliable transportation. Complies with accepted professionals standards and practice.

Pay Rate:
$55.00 per visit
