Registered Nurse (RN) - Home Health, Per Visit Distinctive Home Health, Inc. is a licensed and ACHC-accredited home health agency based in La Crescenta. We are looking for RNs to join our contractor team on a per-visit basis, serving patients across Los Angeles County and surrounding areas. Why work with us Set your own availability. What you'll do Complete OASIS assessments (SOC, ROC, Recert, Transfer, Discharge) Perform skilled nursing visits including wound care, IV therapy, catheter and drain care, and medication management Develop and update the plan of care in coordination with the physician Document visits accurately and on time in WellSky Communicate with our office on patient status changes and physician orders Service areas Los Angeles, Orange, Riverside, San Bernardino, and Ventura counties Apply here and we'll follow up with next steps.
