•78/hour Job Summary Sharp Grossmont Hospital is seeking experienced Registered Nurses (RNs) with Case Management experience to provide safe, individualized, and patient-centered care. The RN Case Manager will assess patient needs, develop and implement care plans, coordinate services, educate patients and families, and collaborate with the interdisciplinary healthcare team Required Qualifications Active California RN License
•Required. CPR Certification
•Required. Minimum 3 years of recent RN experience . Minimum 2 years of recent experience in a specialty area , preferably Case Management. Associate Degree in Nursing (ADN) or higher. Strong patient assessment, care planning, implementation, and evaluation skills. Excellent communication, documentation, and care coordination skills. Ability to work collaboratively with an interdisciplinary healthcare team. English proficiency
