Monday-Thursday from 5:00 PM to 8:30 AM, and Friday 5:00 PM to Monday 8:30 AM.
Staffing Goal:
We aim to hire at least 3 staff members to rotate on-call coverage and holiday shifts for our full-time team.

Active Care Hourly Rate:
$50-$55/hour
Time Tracking:
Calls lasting under 15 minutes per hour are covered by the stipend. Any time spent beyond that is added as hourly pay on their timesheet. Please let me know if your system handles this differently. We are seeking an experienced and compassionate On-Call Registered Nurse (RN) to support our patients and families throughout Lancaster and the surrounding areas. The On-Call RN plays an important role in ensuring our patients have access to skilled nursing support when needs arise outside of regular business hours. This position is ideal for an RN who is confident working independently, has strong clinical judgment, and remains calm and compassionate during urgent or sensitive situations. Responsibilities Respond to patient and caregiver calls during assigned on-call hours. Make PRN and urgent nursing visits as needed. Assess changes in patient condition and provide appropriate nursing interventions. Provide symptom and pain management in accordance with the patient's plan of care and physician orders. Communicate with physicians and other members of the interdisciplinary team regarding changes in condition and patient needs. Provide education, emotional support, and guidance to patients and families. Assist patients and families during end-of-life changes and provide support at the time of death as needed. Complete timely and accurate clinical documentation for all calls, visits, assessments, and interventions. Coordinate with pharmacies, DME providers, facilities, and other resources when necessary to address after-hours patient needs. Ensure continuity of care by communicating relevant updates to the daytime clinical team. Qualifications Current and unrestricted California Registered Nurse (RN) license . Previous hospice, home health, palliative care, or acute care experience preferred . Strong assessment and critical-thinking skills. Experience with pain and symptom management preferred. Ability to work independently and make sound clinical decisions in the field. Compassionate and confident communicating with patients and families during end-of-life situations. Strong organizational, communication, and documentation skills. Reliable transportation and ability to travel throughout the assigned service area.
