• Skills Current CA state license to practice as a Licensed Vocational Nurse (LVN) in California. A minimum of one (1) year home health care experience is strongly preferred. Current

BLS/ CPR

Certification Experience in caring for patients with G-Tubes, Trachs, and ventilators. Works in patients home in various conditions; ability to work a flexible schedule. Able to travel to prospective patient's place of residence. Able to stoop and bend; must be able to lift and transfer patients.

Responsibilities:

Provide home-based care to patients with illnesses, injuries and chronic diseases—often after hospital discharge—under the supervision of an RN(Registered Nurse) Licensed Vocational Nurse ( LVNs)will be given a detailed plan of care instructed and administered by RN Case Manager Care consists of monitoring patient status , maintaining activities of daily living and administering medications. Take patient vitals, do assessments, document progress and report changes to RN supervisor. Assist RN in carrying out patient's plan of care and teaching patient self-care techniques Collaborates with physicians, other agency staff and contract services to coordinate and implement the Plan of Care. Completes all required documentation in accordance with the standards of the agency, and regulatory requirements.
