Job Description Perform initial and ongoing patient assessments (including OASIS) and develop individualized care plans Administer medications and treatments per physician orders Perform wound care, including cleaning and re-dressing as needed Measure and monitor vital signs, and identify/report changes in patient condition Coordinate with physicians, LVNs, HHAs, and other care team members on treatment plans Supervise and delegate tasks to LVNs and HHAs within their scope of practice Maintain accurate, compliant clinical documentation and progress reports Qualifications Strong initiative to keep the agency in compliance Well organized, self-motivated, and able to work independently under the supervision of the Director of Nursing Excellent organization, communication, and follow-up skills Excellent computer skills (EMR documentation experience preferred) Requirements Valid CA RN license Valid CA drivers license Grow With Us RNs at Get Well Home Health have a clear path to grow into leadership. Field RNs can move into a part-time QA RN role, and from there into a full-time QA RN position overseeing chart review and compliance.
