Requirements:

2-year acute Hospital Case Manager experience performing Care Coordination, Discharge Planning in a very fast paced environment.

Weekends Required:

2 weekends every 6 weeks Float between like units? Yes, based on need Locals accepted-

Up to $2,700.00 per week
