Weeks Position Type:
Travel/Contract Required Qualifications Current and active Registered Nurse (RN) license . Minimum 2 years of acute-care hospital Case Manager experience . Experience performing care coordination and discharge planning . Strong knowledge of hospital-based case management processes. Excellent communication, organization, and critical-thinking skills. Ability to work collaboratively with physicians and multidisciplinary healthcare teams. Strong documentation and time-management skills.
