Help families living with dementia get care in days, not months. Caring for someone with dementia is exhausting, and the wait for help can feel endless. Thalpos changes that. We coordinate in-home dementia care for families enrolled in Medicare's GUIDE Model, working alongside dementia care practices and a growing network of Care Companions who provide respite care, home safety assessments, and telehealth support in the home. Our promise to every family is simple: a matched Care Companion within days. We're looking for a Registered Nurse to own that promise. What you'll do Lead matching. Pair families with the right Care Companion based on location, availability, and the family's needs, across respite, home safety assessment, and telehealth visits. Own the daily rhythm. Start each morning by reviewing every open case, oldest first, and keep each one moving until it's matched. See every match through to the first visit. Follow up with Care Companions on anything not yet scheduled, and solve problems before a family ever notices them. Protect quality. Make sure every Care Companion is fully vetted and ready before they're matched, and act quickly when a match isn't working. Spot coverage gaps early. Partner with our recruiting team to grow the network in areas where families are waiting. Advocate for families. Work with our Client Experience team so every concern is heard and resolved quickly. Make the process better. Own our matching playbook, train fellow nurses on it, and work with our engineering team to build faster, smarter tools. What you bring An active, unrestricted RN license in CA, or a multistate compact license 2+ years in home health, care coordination, case management, or geriatric care Experience supporting people living with dementia and their caregivers Warmth and confidence on the phone, and the habit of always closing the loop The organization to keep many open cases moving at once without dropping any Comfort with web-based tools and a curiosity about improving them Bonus points Familiarity with Medicare programs, especially the GUIDE Model Experience scheduling or supporting home care aides or caregivers Experience working across multiple states Why Thalpos Mission-driven work that makes a real difference for families every week A nursing role focused on coordination and impact, not bedside shifts The chance to shape how a fast-growing care network operates Thalpos is an equal opportunity employer.
