Schedule:
Once per week, daytime hours. Specific days and hours can be discussed based on program needs and candidate availability Crown Community Health Center is seeking a qualified Registered Nurse (RN) to provide nursing services in a Community Mental Health Center / Partial Hospitalization Program setting. This is a part-time/per diem position requiring nursing services once per week . The RN will work with adult clients experiencing mental health symptoms, medical conditions, functional limitations, medication concerns, and difficulty maintaining health and wellness routines. The RN will provide nursing assessment, health education, medication-related monitoring, coordination with the treatment team, and documentation of client health status and progress. Responsibilities Provide professional nursing services to adult
CMHC/PHP
clients. Complete nursing assessments and identify medical or health-related needs. Monitor clients for changes in medication side effects, safety concerns, and functional limitations. Provide health education related to medication compliance, chronic disease management, nutrition, hydration, sleep hygiene, fall prevention, infection prevention, and overall wellness. Facilitate nursing-focused groups as appropriate. Communicate client health concerns to the psychiatrist/PMHNP, treatment team, and administration when needed. Assist with treatment planning by identifying nursing problems, goals, interventions, and client progress. Document client participation, symptoms, health concerns, progress, and response to nursing interventions. Requirements Current and valid California Registered Nurse license required. Ability to work with adults experiencing depression, anxiety, psychosis, bipolar disorder, cognitive limitations, and chronic medical conditions. Strong documentation and communication skills. Ability to work as part of a multidisciplinary treatment team. CPR/BLS certification preferred. Work Environment This position is located in a structured outpatient mental health program. Services are provided during daytime program hours, once per week.

$40.00 - $54.72 per hour Expected hours: 4.0 per week
