Job Overview We are seeking a dedicated and compassionate Registered Nurse (RN) to provide high-quality nursing care within a Skilled Nursing Facility (SNF). This role involves delivering comprehensive patient assessments, administering medications and treatments, and coordinating care plans to promote optimal health outcomes for our residents. Duties Conduct thorough patient assessments, including vital signs, physical examinations, and health history reviews, to develop individualized care plans. Administer medications, including injections and IV infusions, following prescribed protocols while ensuring patient safety and comfort. Monitor patients' conditions continuously, recognizing signs of acute pain, infection, or deterioration, and respond promptly. Collaborate with interdisciplinary teams to coordinate discharge planning, therapy sessions, and ongoing treatment strategies. Maintain accurate medical documentation in compliance with HIPAA regulations and facility policies using EMR (Electronic Medical Record) systems such as Epic or Cerner. Provide specialized care for residents with complex needs such as dementia or memory care requirements. Assist with procedures including suturing, catheterization, ostomy care, and wound management. Educate patients and their families on health management strategies, medication administration, and post-discharge instructions. Ensure infection control standards are upheld through aseptic techniques and proper sterilization procedures. Participate in quality improvement initiatives aligned with NCQA standards and other regulatory requirements. Requirements Valid RN license in the state of practice with current certification in Basic Life Support (BLS). Minimum of two years' experience working in a long-term care facility or hospital setting; experience in skilled nursing or sub-acute care preferred. Proficiency with EMR/EHR systems such as Epic or Cerner; familiarity with ICD coding (ICD-10/ICD-9) is advantageous. Strong clinical skills including IV insertion, medication administration, tube feeding management, and laboratory specimen collection. Knowledge of geriatrics, dementia care, hospice & palliative medicine principles, and trauma medicine is highly desirable. Ability to perform physical examinations accurately and assess patient physiology effectively. Excellent communication skills for effective triage, patient education, and interdisciplinary collaboration. Experience working with individuals with developmental disabilities or behavioral health needs is a plus. Demonstrated understanding of infection control practices and OSHA standards within healthcare environments.
