In this vital role, you will be responsible for coordinating the admission process for patients entering hospice care, ensuring a smooth transition from hospital, outpatient, or home settings to end-of-life care. Your expertise in patient assessment, medical records review, and discharge planning will help deliver personalized, high-quality hospice services. This position offers an exciting opportunity to make a meaningful difference in patients' lives during their most vulnerable moments while working within a collaborative and supportive environment. Responsibilities Conduct comprehensive patient assessments including vital signs, Hospice evaluation, and medical history review to determine eligibility for hospice services. Collaborate with healthcare providers, families, and case management teams to facilitate seamless admission processes. Review medical records and diagnostic evaluations to verify clinical criteria and ensure compliance with hospice guidelines. Coordinate intake procedures, including scheduling visits, explaining services, and addressing patient and family questions with clarity and empathy. Manage discharge planning by evaluating post-acute care needs and facilitating referrals to appropriate outpatient or inpatient services. Maintain detailed documentation of patient interactions, assessments, and admission notes in compliance with HIPAA regulations. Evaluate patient needs for appropriate level of care such as inpatient, outpatient, or clinic-based services. Experience Candidates should possess at least 1yr working in the Hospice field
