Per Visit Rates ($60-$180) + Mileage •rates are determined by experience. rates are per visit, not per hour• Job Overview We are seeking a compassionate and highly skilled Hospice Case Manager RN to join our dedicated team. In this vital role, you will coordinate and deliver comprehensive end-of-life care, ensuring patients and their families receive the highest quality support during challenging times. Your expertise in critical care, hospital medicine, and post-acute care will drive personalized care plans, facilitate smooth transitions across care settings, and uphold the dignity and comfort of each patient. This position offers an exciting opportunity to make a meaningful difference in patients' lives while working within a dynamic, team-oriented environment. Duties Conduct thorough patient assessments, including vital signs, physiology evaluation, and medical history review to determine eligibility for hospice services. Develop and implement individualized care plans that incorporate symptom management, pain control, and psychosocial support aligned with patient wishes. Coordinate interdisciplinary team activities such as discharge planning, diagnostic evaluation, and utilization management to ensure seamless patient transitions across inpatient, outpatient, and post-acute settings. Provide expert guidance on complex cases involving infectious disease care, behavioral health issues, geriatrics, pediatrics, or trauma-related conditions like Level I or Level II trauma center experiences. Manage patient records meticulously in compliance with HIPAA regulations while documenting medical records, intake information, and clinical notes accurately. Collaborate with primary care providers and hospital staff to facilitate effective communication regarding patient care goals and treatment updates. Educate patients and families about disease processes, medication management, and available community resources to promote informed decision-making. Qualifications Current RN licensure with active registration; experience in critical care environments such as
ICU, PICU
(Pediatric Intensive Care Unit), or emergency room settings is highly preferred. Demonstrated expertise in hospital medicine, medical-surgical nursing, or post-acute care environments; experience in managed care or utilization management is a plus. Strong knowledge of physiology, medical terminology, diagnostic evaluation procedures, and discharge planning processes. Proven ability to perform comprehensive patient assessments including vital signs and functional evaluations within acute or outpatient settings. Experience working with diverse populations such as geriatrics or pediatrics; familiarity with behavioral health considerations is advantageous. Excellent communication skills for effective collaboration with multidisciplinary teams while maintaining strict adherence to HIPAA standards. Ability to navigate complex cases involving trauma centers or infectious disease scenarios confidently; prior OR experience or involvement in emergency medicine is beneficial.
