Job Summary We are seeking a highly skilled Registered Nurse with extensive home health office experience to join our dynamic team. The ideal candidate will possess a strong background in clinical documentation, utilization management, and medical coding, with a focus on providing exceptional patient care coordination within a home health setting. Responsibilities Perform comprehensive review of medical documentation to ensure accuracy, completeness, and adherence to clinical documentation standards. Coordinate with interdisciplinary teams to facilitate discharge planning, case management, and patient care transitions in compliance with CMS and state healthcare regulations. Utilize EMR/EHR systems to manage electronic health records efficiently and securely. Conduct utilization review and management activities to optimize patient care while maintaining regulatory compliance for Medicare, Medicaid, and private insurers. Apply medical coding skills including CPT, ICD-9, ICD-10, DRG, and HEDIS measures for accurate billing and reporting purposes. Maintain knowledge of long-term care regulations, hospital experience standards—including ICU and trauma center protocols—and acute care practices relevant to home health services. Collaborate with physicians and healthcare providers to ensure proper discharge planning and case management aligned with Medicare regulations and long-term care policies. Requirements Valid Registered Nurse (RN) license with current state licensure. Proven experience working in home health office environments or related outpatient settings. Strong knowledge of EMR systems. Demonstrated expertise in medical coding (CPT, ICD-9/10), utilization management, and clinical documentation review. Familiarity with CMS guidelines and Medicare regulations. Experience with long-term care regulations, discharge planning processes, and inpatient or ICU settings preferred. Excellent understanding of medical terminology, anatomy, physiology, and healthcare policies including HIPAA and state-specific regulations. Proficiency in Microsoft Office Suite for documentation review and reporting purposes. Ability to work collaboratively within multidisciplinary teams while maintaining attention to detail in documentation standards.
