Active Registered Nurse (RN) license in the state of practice Basic Life Support (BLS) / CPR (BLS, HS-BLS, or RQI from approved vendor) Advanced Cardiovascular Life Support (ACLS) Pediatric Advanced Life Support (PALS) (AHA) NIH Stroke Scale (NIHSS) certification Crisis management certification (TEAM, TEAM-A, or CPI)

REQUIREMENTS

Registered Nurse, ER RN-CA Certifications:

ACLS(Copy needed); BLS(Copy needed); PALS(Copy needed)

SSN Required DOB Required Work History:

Should not be more than 30 days of gap in last 1 year

References:
