Job Description

By The Bay Health, a non-profit established in 1975, set the standard for hospice in the U.S. by emphasizing the role of the patient in making important medical decisions. Our spectrum of home-based services now includes Skilled Home Health Care, Palliative Care, Adult Hospice Care and Pediatric Care. Our team approach strives to address practical, social, emotional and spiritual aspects of care, with the goal to maximize quality of life for our patients, caregivers and families. This position offers competitive pay, generous benefits, shift differential and a supportive work environment. We are currently seeking an RN to be a part of our team. The ideal candidate will provide coverage for After-Hours to cover patient/staffing needs and processes admissions for eligible hospice patients in coordination with Team Manager.
Schedule:
Thursday - Monday.; 4:00pm-12:30am.
Location:
Marin County with additional coverage to San Francisco & Sonoma Counties as needed Essential Duties & Responsibilities include (but are not limited to): Make in-person nursing visits to patients as assigned, or as indicated. Provide continuing care to patients/families as requested and in compliance with agency policies and procedures and applicable federal and state regulations. Use the nursing process to assess, plan, intervene and evaluate following patient/family needs and goals. Provide effective management of symptoms, including prompt escalation to providers or others as indicated. Coordinate patient/family services and prioritization of needs with the members of the transdisciplinary team or outside providers/services.
Education/Experience :
Associate's or Bachelor's degree in nursing from an accredited School of Nursing Experience in acute care/ home health and/or hospice setting preferred
Certificates, Licenses, Registrations :
Current license as a registered nurse in the State of California. Public Health Nursing certification highly desirable. HPNA certification preferred.
Pay Range:
