Position:
Registered Nurse (RN)
Location:
Sonoma Post Acute, Sonoma, CA Responsibilities:
Assess, plan, implement, and evaluate nursing care for residents in accordance with established policies and procedures. Administer medications and treatments as prescribed by physicians. Monitor residents' health status and respond promptly to changes in condition. Collaborate with interdisciplinary team members to develop and implement individualized care plans. Provide education and support to residents and their families. Maintain accurate and timely documentation of resident care and observations. Ensure compliance with regulatory standards and guidelines. Foster a positive and supportive environment for residents and staff.
Requirements:
Current licensure as a Registered Nurse (RN) in California. Compassionate and patient-centered approach to care. Strong clinical skills and knowledge of nursing principles and practices. Excellent communication and interpersonal skills. Ability to work effectively in a fast-paced environment and collaborate with a multidisciplinary team. Prior experience in a skilled nursing or long-term care facility preferred, but not required. How to
