Our interdisciplinary team focuses on comfort, dignity, and quality of life for patients and families facing life-limiting illness. We are seeking a skilled and compassionate Registered Nurse (RN) to join our growing hospice team. Position Summary The Hospice RN Case Manager / Visiting Nurse is responsible for providing skilled nursing care to hospice patients in their place of residence. The RN collaborates with the interdisciplinary team to ensure comprehensive, individualized care that supports comfort and symptom management. Essential Responsibilities Perform comprehensive nursing assessments of hospice patients Develop, implement, and update individualized Plans of Care Manage pain and symptoms in collaboration with the hospice physician Provide patient and family education regarding disease progression, medications, and end-of-life care Administer medications, treatments, and wound care as ordered Coordinate care with social workers, chaplains, CNAs, and other team members Participate in IDT (Interdisciplinary Team) meetings Ensure timely and accurate documentation in compliance with Medicare, California regulations, and hospice Conditions of Participation Provide emotional support and advocacy for patients and families Participate in on-call rotation as assigned Qualifications Active California RN license (required) Minimum 1 year RN experience (hospice or home health preferred) Strong assessment and critical thinking skills Excellent communication and documentation abilities Compassionate and patient-centered approach Valid driver's license, reliable transportation, and auto insurance CPR certification (BLS required) Preferred Qualifications Hospice or palliative care experience Case management experience EMR experience
