Required Position Summary We are seeking an experienced Hospice Admission Visit Registered Nurse (RN) to provide compassionate, patient-centered care to individuals and their families throughout the hospice admission process. The Hospice Admission Visit RN is responsible for admitting patients to hospice services, conducting comprehensive assessments, and developing an individualized plan of care based on the patient's physical, psychosocial, environmental, safety, and developmental needs. The RN will document patient assessments, observations, interventions, and care needs while coordinating services with the multidisciplinary hospice team. This position also participates in the hospice program's Quality Assessment and Performance Improvement (QAPI) program and conducts follow-up visits as needed. Key Responsibilities Conduct hospice admission visits and admit patients to hospice services. Perform comprehensive assessments of the patient's physical, psychosocial, environmental, safety, and developmental needs. Develop an individualized plan of care based on the patient's assessment and identified needs. Document patient assessments, observations, interventions, and care provided accurately and timely. Coordinate patient care with the multidisciplinary hospice team. Communicate effectively with patients, families, caregivers, physicians, and other healthcare professionals. Provide education and emotional support to patients and families regarding hospice services and the plan of care. Conduct follow-up visits as needed based on patient and family needs. Monitor changes in patient condition and communicate significant findings to the appropriate members of the care team. Participate in the hospice program's Quality Assessment and Performance Improvement (QAPI) activities. Maintain compliance with applicable hospice standards, policies, procedures, and documentation requirements. Provide compassionate, respectful, and professional care to patients and their families.
