Job Description

Pathways Home Health & Hospice is seeking an experienced Hospice RN to provide compassionate, patient-centered care in a home-based setting.

Facility:

Pathways Home Health & Hospice Pay:

$60/hour

Responsibilities:

Admit patients to hospice services and complete assessments. Develop individualized plans of care. Conduct follow-up visits and document patient care. Coordinate care with the multidisciplinary hospice team. Requirements Active RN license 1+ year of Hospice experience required Strong clinical assessment and documentation skills. HCHB (Homecare Homebase) experience preferred. Schedule Option 1: Sunday-Thursday Option 2:

Tuesday-Saturday Hours:

10 AM-7 PM or 11 AM-8 PM Schedule may be flexible based on patient availability.
