This role provides direct patient care to hospice patients, supporting comfort-focused, end-of-life care in the field. Responsibilities Conduct patient evaluations and assessments for hospice patients Develop and manage individualized care and symptom management plans Prescribe and manage medications for comfort-focused care Collaborate with physicians, RNs, social workers, and the interdisciplinary hospice team Travel to patient locations as needed for visits Document patient encounters and care plans accurately and timely Ensure compliance with organizational policies, safety standards, and regulatory requirements Requirements Active California Nurse Practitioner license required Hospice or palliative care experience preferred Active DEA registration required Reliable transportation required Schedule Details Monday
