Responsibilities Conduct comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform treatment plans. Administer medications and injections accurately following prescribed protocols, including IV infusions and medication management. Perform specialized procedures such as suturing, catheterization, tube feeding, and ostomy care with precision and aseptic technique. Monitor patients' conditions continuously using advanced equipment like ventilators and cardiac monitors; respond swiftly to changes in status. Document all patient interactions thoroughly in electronic health record (EHR) systems such as Point-Click Care, ensuring compliance with HIPAA regulations. Collaborate with interdisciplinary teams—including physicians, therapists, and case managers—to coordinate comprehensive care plans. Educate patients and their families on health management strategies, including pain management and post-discharge instructions. Qualifications Valid RN license in good standing with relevant state licensing boards. Hands-on experience with EMR/EHR systems like Point Click Care; familiarity with medical documentation standards including MDS and CPT coding is advantageous. Skilled in performing complex procedures such as IV insertion. Ability to work effectively with diverse patient populations including geriatrics, individuals with developmental disabilities or dementia care needs.
