Registered Nurse - Childrens Mental Health

Pathways to Wellness, a community-based outpatient mental health clinic dedicated to supporting individuals with mental health challenges seeks a part-time Children's Registered Nurse to provide services to children, youth, and families through compassionate, culturally responsive, and evidence-based care. Our multidisciplinary team works collaboratively to promote wellness, early intervention, and continuity of care for every child and family we serve. The Registered Nurse (RN) is a vital member of our children's behavioral health team and plays a key role in supporting the medical and psychiatric needs of our pediatric population. This position ensures smooth coordination of care, including communication between families, psychiatric providers, therapists, and pediatricians. The Children's RN is a key player in coordinating incoming psychiatric inpatient hospital referrals to our outpatient clinics and will coordinate and collaborate across all three clinic sites within our agency to ensure seamless transitions of care and consistent service delivery. 2-3 days per week. Once a month travel to Oakland clinic. Duties Collect and record vital signs and health information during intake and follow-up visits. Assists medical providers with the completion of medication check visits on returning children as needed. Coordinate medication management between medical providers, pharmacies, and caregivers. Administer Injections Collaborate with pediatricians and external healthcare providers to ensure continuity of care. Facilitate warm handoffs for children referred from inpatient hospitals, ensuring seamless transitions into outpatient services. Provide health education and wellness support to children, caregivers, and families Monitor and document clients' physical and mental health status. Participate in treatment team meetings, case reviews, and clinical coordination across all three agency clinics. Maintain compliance with all state, federal, and county documentation standards Support crisis intervention, safety planning, and follow-up care. Conduct comprehensive patient assessments to determine care needs and develop individualized care plans. Collaborate with multidisciplinary teams to facilitate effective discharge planning and transitions of care. Monitor patient progress and adjust care plans as necessary based on clinical evaluations and patient feedback. Provide education to patients and their families regarding treatment options, health management, and available resources. Maintain accurate medical records and documentation in compliance with HIPAA regulations and medical terminology standards. Utilize critical thinking skills to perform diagnostic evaluations and manage acute care situations effectively. Engage in utilization management activities to ensure appropriate resource allocation and quality of care. Advocate for patients by navigating complex healthcare systems, including managed care, outpatient services, and post-acute care resources. Requirements Current California Registered Nurse (RN) license in good standing Minimum 3-5 years of experience in behavioral health, pediatrics, or community health preferred Knowledge of psychiatric medications, side effects, and monitoring requirements Strong communication and family engagement skills Ability to manage care coordination across multiple clinic sites Familiarity with Medi-Cal and county behavioral health systems and documentation
