Sprout Organization runs Recuperative Care, a Medi-Cal (CalAIM Community Supports) funded program for people discharged from the hospital who have nowhere safe to go to recover. The Registered Nurse provides the clinical monitoring at the center of this program — assessing members, reconciling medications, directing non-clinical staff, and determining when a member's condition exceeds what this setting can safely hold. About the Role This is a coordinated clinical care model at the minimum-service level. RN presence is approximately 2 hours per site per day and does not constitute continuous nursing care. The RN works independently, without another nurse on-site, and must exercise sound clinical judgment. Responsibilities Conduct daily clinical rounds and document nursing assessments in SOAP form (Tebra) Reconcile member medications and verify secure self-storage Triage escalations from Support Specialists and determine response level Determine and initiate hospital transfers when needed Participate in paid after-hours on-call rotation Complete and maintain member care plans, reviewing/signing at least every 14 days Assess and document discharge readiness Provide daily written monitoring instructions to non-clinical staff and conduct competency training Coordinate with outside providers, clinics, hospitals, and pharmacies Provide member education on condition management and follow-up care Qualifications Current, unrestricted California RN license Minimum two years of clinical nursing experience Current BLS certification Valid CA driver's license and reliable transportation Strong independent clinical judgment Preferred Community health, home health, correctional, shelter-based, or street medicine background Wound care experience Experience with substance use or behavioral health populations Bilingual Pre-Employment Requirements Must be 18+ Background check required before unsupervised member contact Negative TB clearance within 12 months of start date Signed Staff Scope Acknowledgment and mandated reporter acknowledgment

Schedule:

Daily clinical rounds, approximately 2 hours per site per day, within an 11:00 AM-3:00 PM window. Includes a paid on-call rotation.

Pay:

$60.00 per hour Expected hours: 12.0 - 16.0 per week
