Registered Nurse (RN) - Floor Nurse North Shore Health and Rehab | Loveland, CO Position Summary The Floor Nurse (RN) is responsible for providing direct, high-quality nursing care to residents at North Shore Health and Rehab, in accordance with physician orders, facility policy, and state and federal regulations governing skilled nursing facilities. The Floor Nurse supervises Licensed Practical Nurses (LPNs) and Certified Nurse Aides (CNAs) on assigned shifts, coordinates resident care, and serves as a key point of contact for residents, families, and the care team. Essential Duties and Responsibilities Conduct ongoing assessments of residents' physical, emotional, and psychosocial status, and document findings accurately Administer medications and treatments in accordance with physician orders and facility policy Develop, implement, and update individualized care plans in collaboration with residents, families, and the interdisciplinary team Supervise and direct the work of LPNs and CNAs, providing guidance and oversight consistent with each role's scope of practice Respond promptly to changes in resident condition, notify physicians and family members as needed, and initiate appropriate interventions Maintain accurate, timely documentation in the electronic medical record Communicate effectively with residents, families, physicians, and other healthcare providers Participate in care conferences, quality assurance initiatives, and facility in-service training Ensure proper infection control practices, including hand hygiene and isolation precautions Monitor and maintain appropriate levels of medications and supplies Respond to emergencies, including initiating CPR and other life-saving measures as required Promote a safe, clean, and dignified environment that supports residents' rights and quality of life Perform other nursing duties as assigned by the Director of Nursing or Administrator Qualifications Current, unencumbered license as a Registered Nurse (RN) in the State of Colorado Graduate of an accredited school of nursing (ADN or BSN) Current CPR/BLS certification (or willingness to obtain within a specified timeframe) Prior experience in a skilled nursing, long-term care, or rehabilitation setting preferred; new graduates welcome to apply Strong clinical assessment, critical thinking, and decision-making skills Excellent communication and interpersonal skills Proficiency with electronic medical record (EMR) systems Ability to pass a background check and drug screening Physical Requirements Ability to stand, walk, bend, and move throughout the facility for extended periods during a shift Ability to lift, transfer, or assist with repositioning residents, with or without assistive devices Manual dexterity for clinical tasks such as medication administration and wound care Visual and auditory acuity sufficient to monitor resident status and respond to alarms or call lights Work Environment This position is based on-site at North Shore Health and Rehab in Loveland, Colorado. Shifts may include weekends, holidays, and on-call rotations as scheduled by the Director of Nursing.
