An opportunity is available for an experienced Nurse Practitioner to perform home health risk assessments in Montrose County, Colorado. This per diem role provides flexibility for those looking to design their own schedule while delivering high-quality care to Medicare Advantage plan members within the community.

Schedule/Hours:

Flexible shifts between Monday and Sunday Create your own schedule Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

Conduct comprehensive home health risk assessments, each approximately 45 minutes Assess patients prior and current medical history, medical conditions, and medication use Provide clear education to members about their health conditions and prescribed medications Identify care gaps and recommend solutions to promote health and wellness No responsibility for creating treatment plans or prescribing medications

Qualifications:

Active Nurse Practitioner license in Colorado Board Certification via AANP or ANCC Current BLS/CPR certification Minimum 6 months of recent NP experience preferred Master of Science in Nursing (MSN) from an accredited program
