This position will begin as PRN, with the opportunity to transition to a full-time position within approximately 30 days, based on successful onboarding, training, performance, and business needs. Service Area Westminster and surrounding communities Approximately 15-20-mile service radius in various directions Local travel between client locations is required

Potential transition to full-time within 30 days Responsibilities Provide compassionate, high-quality nursing care to clients in their homes Complete nursing assessments and monitor clients' conditions Develop and follow individualized plans of care Administer medications and treatments as ordered Communicate effectively with clients, families, physicians, and the care team Document services accurately and timely Identify changes in condition and report concerns appropriately Provide education and support to clients and their families Maintain compliance with all applicable healthcare regulations and company policies Qualifications Current and active RN license in Colorado Previous home health, hospice, or community-based nursing experience preferred Strong clinical assessment and communication skills Compassionate, dependable, and patient-centered Ability to work independently and manage multiple client needs Reliable transportation and willingness to travel within the assigned service area Ability to complete required documentation accurately and on time Why Join Our Team?
