Columbine Commons Health and Rehab in Windsor is seeking a dedicated and compassionate Long-Term Care Skilled Nursing Facility Nurse to provide exceptional patient care in our dynamic healthcare environment. This role involves managing a diverse patient population, including individuals with complex medical needs such as dementia. The ideal candidate will possess extensive clinical expertise across various specialties, including geriatrics, behavioral health, acute care, and post-acute services. Experience with electronic health records systems such as Epic and Point Click Care is highly desirable. The licensed Nurse will collaborate with interdisciplinary teams to ensure comprehensive care delivery while adhering to HIPAA regulations and quality standards like NCQA. Responsibilities Conduct thorough patient assessments, including vital signs, physical examinations, and physiological evaluations. Administer medications and injections accurately following aseptic techniques and medication protocols. Manage IV infusions, catheter care, ostomy care, and ventilator support for patients with complex needs. Utilize EMR/EHR systems for accurate medical documentation, coding (ICD-10/ICD-9), and case management. Coordinate discharge planning and collaborate with families for post-acute care or hospice & palliative medicine services. Ensure infection control practices are strictly followed to maintain a safe environment for patients and staff. Engage in ongoing education related to medical terminology, CPT coding, HIPAA compliance, and current nursing best practices. Experience Proven clinical experience in long-term care facilities or nursing homes is essential. Prior experience working with elderly populations in geriatrics or dementia/dementia care settings is highly preferred. Critical care experience such as ICU or emergency room background is advantageous. Familiarity with EMR/EHR systems like Epic and PCC is preferred. Experience administering injections, performing phlebotomy/venipuncture Knowledge of infectious disease protocols, trauma medicine (including trauma transport), and post-operative or outpatient surgical care adds value. Additional experience in behavioral health, pain management, or case management is a plus.
