2996662139-2 Apply for this job now Job Description Hartford Healthcare seeks a Weekend OASIS Registered Nurse to provide comprehensive in-home assessments and skilled nursing care to adult and geriatric patients across our Connecticut service area. In this visiting nurse role, you will complete accurate, timely OASIS assessments, develop individualized care plans, administer treatments, and educate patients and families on disease management and medications. You'll collaborate with an interdisciplinary team, leverage advanced technology for documentation, and practice within a culture that values compassion, teamwork, and ongoing professional growth. Responsibilities Conduct comprehensive in-home OASIS assessments for adult and geriatric patients on weekend shifts Develop, implement, and update individualized patient care plans based on assessment findings Provide skilled nursing interventions, including medication administration, wound care, and symptom management Document all care and OASIS data accurately and promptly in the electronic health record system Educate patients and families on disease processes, medication regimens, and safe home care practices Coordinate services and communicate changes in patient status with the interdisciplinary care team Prioritize visits and manage weekend caseload and routing efficiently Ensure compliance with regulatory, agency, and quality standards for home health care Participate in case conferences and contribute to continuous quality improvement initiatives Maintain patient safety and infection control standards during all home visits Required Skills OASIS assessment and documentation Home health nursing Care planning and coordination Wound care and medication management Electronic health record (EHR) documentation Patient and family education Time and route management Interdisciplinary team collaboration Clinical triage and critical thinking Infection control practices
