In this role, you will coordinate care across the continuum, develop and monitor patient-centered care plans, and facilitate safe transitions between inpatient, outpatient, and community settings. You will collaborate closely with physicians, nurses, and interdisciplinary teams to optimize resource utilization, reduce readmissions, and support families through education and advocacy. Responsibilities Assess patients' clinical and psychosocial needs to develop individualized care plans Coordinate care transitions between hospital, outpatient, and community settings Collaborate with physicians and interdisciplinary teams on discharge planning Advocate for patients and families, providing education and support Monitor outcomes, utilization, and readmission risk, adjusting plans as needed Document assessments, plans, and progress in the EMR accurately and timely Ensure appropriate use of resources and alignment with payer requirements Identify and address barriers to care, including social determinants of health Participate in quality improvement and population health initiatives Maintain compliance with regulatory, ethical, and organizational standards Required Skills Clinical assessment Care coordination Discharge planning Utilization management Electronic medical records (EMR) Interdisciplinary collaboration Patient and family education Knowledge of payer and insurance requirements Data and outcomes tracking Time management
