Job Description

An exciting opportunity is available for an experienced and compassionate Registered Nurse specializing in Home Health to deliver patient-focused care in Shelton, CT. This contract position requires a skilled professional who thrives in an autonomous, field-based setting, dedicated to enhancing patient outcomes through individualized care and clinical excellence.
Schedule:
Monday Friday, 8:00 AM 5:00 PM 13-week contract
Caseload:
2532 points per week (typically 35 patient visits per day) Daily travel within assigned territory (mileage reimbursement provided)
Qualifications:
Active RN license (in good standing) in the state of practice Minimum 1 year recent home health nursing experience (field-based strongly preferred) Proficient with Home Care Home Base (HCHB) electronic documentation Strong working knowledge of OASIS assessment tool Reliable transportation, valid drivers license, and active auto insurance Excellent clinical assessment, organization, and communication abilities Ability to work independently and manage a flexible travel schedule BSN preferred Familiarity with Medicare and home health compliance standards
Responsibilities:
Perform comprehensive OASIS assessments and develop individualized care plans Deliver skilled nursing services including wound care, IV therapy, medication administration, and chronic disease management Educate patients and families regarding disease processes, treatments, and preventive care Monitor and document patient status and coordinate with physicians and interdisciplinary team members Maintain timely and accurate charting in HCHB Attend case conferences, staff meetings, and opportunities for continuing education
