2996670801-2 Apply for this job now Job Description Hartford Healthcare is seeking a compassionate Hospice Admission Nurse to coordinate and deliver high-quality hospice admission services across our network. In this role, you will conduct comprehensive patient assessments, explain hospice services, and develop individualized care plans in collaboration with an interdisciplinary team. You will educate patients and families, ensure smooth transitions from acute or home settings, and document care using our advanced EHR systems. Responsibilities Conduct comprehensive hospice admission assessments for new patients in home and facility settings. Explain hospice philosophy, services, and care options to patients and families with empathy and clarity. Develop and implement individualized admission and initial care plans with the interdisciplinary team. Coordinate transitions from hospitals, clinics, or home care into hospice services. Accurately document assessments, care plans, and interventions in the EHR in a timely manner. Collaborate with physicians, social workers, chaplains, and other clinicians to ensure continuity of care. Monitor initial patient response and communicate changes in condition or needs to the care team. Provide education on symptom management, medications, and safety to patients and caregivers. Ensure compliance with hospice regulations, policies, and quality standards. Participate in team meetings, case conferences, and ongoing professional development activities.
