Responsibilities:

Deliver in-home nursing care, skilled interventions, and comprehensive assessments Accurately complete OASIS documentation and maintain timely records Coordinate with physicians and interdisciplinary teams to implement individualized care plans Perform wound care, manage medications, educate patients and families, and monitor disease progression Communicate patient updates and any changes in condition promptly to the clinical team

Qualifications:

Active Registered Nurse license in good standing At least one year of recent RN experience in a home health setting Proven OASIS documentation skills Familiarity with HomeCare HomeBase (highly preferred) Reliable transportation and valid driver's license Strong organizational skills to manage full-time caseload
