Job Description

Duties The Primary Care Clinic RN Care Manager is a professional registered nurse who is accountable for the management of a high-risk group of patients in Primary Care. The RN is member of a Patient Aligned Care Team (PACT). The PACT teamlet (Patient, Primary Care Provider, RN Care Manager, LPN/Clinical Associate, and Medical Support Associate) facilitates an interdisciplinary approach to high quality, cost effective and customer focused patient-centered health care. The RN Care Manager uses the nursing process and evidence-based practice to collaborate with the interdisciplinary team in developing the patient-driven care plan for life. The RN Care Manager is responsible for collaborating with services internal and external customers in the VA to facilitate care transition in order to effectively meet the patients' needs. The RN Care Manager provides patient and family health education with a focus on self-management, prevention, and wellness, based on the patient's goals. The RN Care Manager demonstrates leadership by serving as an advocate for patients, and a team leader with colleagues as she/he continues to enhance his/her own and the team's professional growth, development, and practice.

Pay:

Competitive salary, regular salary increases, potential for performance awards
