Job Description

Suncoast Hospice of Hillsborough , a member of Empath Health , is seeking an RN Case Manager/RN Float to join the Empath team. The RN Case Manager/RN Float coordinates care and provides nursing services for hospice patients and their families in private homes, assisted living facilities and long term care communities. The RN Float works in all of these areas to provide care as needed on the following schedule Monday - Friday, 8a to 5pm in Hillsborough County. Since 1977, Suncoast Hospice has been honored to serve our neighbors across Pinellas and Hillsborough counties with compassionate end-of-life care. As part of Empath Health, we help those living with advanced illness find comfort, dignity, and meaning, while supporting their families and caregivers with guidance and care every step of the way. What you'll Do Under the direction of a licensed physician, manages discomfort and provides symptom relief using specialized nursing skills related to palliative and end-of-life care. Provides initial and ongoing assessment of the impact of the terminal diagnosis on the patient's physical, functional, psychosocial, and environmental needs and activities of daily living. In collaboration with the other members of the Interdisciplinary Group (IDG); develops implements and updates the individualized plan of care, initiates appropriate preventive and rehabilitative procedures and initiates referrals to other services; requests complimentary services, as needed. Educates patient/resident, family and caregivers regarding disease process and decline, palliative interventions care giving, dying process and safety practices. Supervises LPN's and paraprofessionals providing services to the patient according to regulations. Why Join Empath Health?

What You'll Need Valid Florida license as a Registered Nurse Valid Florida State Driver's License Minimum one year experience in an acute-care Hospital environment providing direct patient care; previous experience as a Hospice RN preferred. Bilingual Spanish/English preferred For field-based positions, employees must have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion. Must provide proof of valid automobile insurance, a copy of which will be placed in the employee's HR file.
