Administrator Job Summary We are seeking a compassionate, organized, and experienced Registered Nurse (RN) Care Coordinator to join our home care team. In this role, you will coordinate and supervise caregivers in the delivery of patient care while ensuring compliance with agency policies and procedures. The RN Care Coordinator is responsible for conducting client assessments, creating and maintaining individualized care plans, supervising caregivers, and maintaining strong relationships with clients and their families. This position plays a key role in ensuring high-quality, safe, and compliant care services for our clients. The RN Care Coordinator will follow all protocols and procedures outlined in the agency's RN Care Coordinator Manual. Key Responsibilities Conduct initial home care visits and perform ongoing reassessments of client needs and progress. Create, implement, and maintain individualized plans of care. Perform reassessments and updates for 60-day care plans. Conduct new client assessments promptly when requested. Coordinate care between clients, families, caregivers, and the agency. Serve as the primary contact for assigned client families and respond to inquiries professionally. Supervise caregivers and provide guidance on proper care techniques. Observe, assess, and document client symptoms and health changes. Monitor patient progress and reactions to care plans or treatments. Educate clients and caregivers as necessary on care plans and treatment approaches. Provide instruction on proper in-home caregiving techniques. Notify the Administrator of any changes in patient condition. Maintain accurate and timely clinical documentation according to agency policy. Ensure all services follow the agency's approved policies and procedures. Be available for communication and/or assessments primarily between 9:00 AM - 5:00 PM Monday through Friday, with communication availability every other weekend or as Nurse rotation dictates. Coordinate with your RN partner (if assigned) to ensure client coverage when needed. Assist with new caregiver orientation training if requested. Perform additional duties within a reasonable scope as assigned by leadership. Qualifications Active Registered Nurse (RN) license in good standing. Minimum of two (2) years of nursing experience as an RN. Ability to build positive and professional relationships with clients, families, and staff. Knowledge and skills required to provide care primarily for geriatric patients, and to a lesser degree pediatric and adult populations. Strong organizational and leadership skills. Excellent verbal and written communication skills. Strong clinical documentation skills. Professional demeanor and appearance. Preferred Experience Experience in home health or home care services. Care coordination or case management experience. Experience supervising caregivers or healthcare staff.
