The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. This role requires a strong understanding of patient care standards, infection control protocols, and the ability to work independently while maintaining effective communication with healthcare teams and families. Duties Conduct comprehensive assessments of patients' health status, including physical, emotional, and social needs. Develop and implement individualized care plans in collaboration with the healthcare team. Administer medications and treatments as prescribed, ensuring adherence to safety protocols and standards. Monitor patients' progress and adjust care plans as necessary based on their evolving needs. Document all patient interactions accurately in compliance with HIPAA regulations. Collaborate with interdisciplinary teams to ensure comprehensive care delivery, including specialists in occupational health when needed. Experience Previous experience in Medicare home health nursing or a similar setting is required. Familiarity with infection control systems and standards is essential. Strong knowledge of Axxess, OASIS and SOC. Strong understanding of HIPAA regulations to maintain patient confidentiality and privacy. Ability to work independently while managing multiple patient cases effectively.
