Case Management Responsibilities:

Educate patients on treatment plans, follow-up instructions, and self-management strategies. Coordinate scheduling, referrals, and follow-up care with clinical and administrative teams. Support care coordination activities across providers to promote continuity of care. Maintain infection prevention practices during patient encounters and procedures.

Qualifications & Skills:

Minimum of 2+ years of experience as an RN. Strong communication and coordination skills. Ability to educate patients effectively and adhere to infection control protocols.

Key Duties:

Work 40 hours per week in 8-hour shifts, providing patient care and care coordination. Ensure seamless communication among healthcare teams and support patient self-management.
