Job Summary We are seeking dedicated Part-Time Registered Nurses (RNs) to join our private duty home health team, working 2 to 3 days per week. The ideal candidates will possess a strong clinical background, excellent patient assessment skills, and a passion for delivering high-quality, personalized care in a home setting. This role offers an opportunity to make a meaningful difference in the lives of seniors and individuals with disabilities by providing expert nursing services tailored to each patient's needs. Duties Conduct comprehensive patient assessments, including vital signs, medication administration, and physical examinations to develop individualized care plans. Administer medications and treatments such as IV infusions, wound suturing, and specialized procedures like botulinum toxin treatments when applicable. Monitor patient progress and document medical records accurately using Wellsky. Provide skilled nursing interventions including catheter care, ostomy management, and infection control practices following HIPAA regulations. Collaborate with physicians and other healthcare professionals to coordinate care plans, discharge planning, and case management strategies. Educate patients and their families about health conditions, medication management, and safety protocols within the home environment. Conduct new hire Orientation with Skills Checks and training Conduct ongoing Case Management Reviews and staff Supervisory Visits in our clients' homes. Skills Proven clinical experience in Home Health, assisted living, skilled care or hospital setting. Strong knowledge of physiology, medical terminology, aseptic technique, and diagnostic evaluation methods. Familiarity with working with individuals with memory care needs; experience in geriatrics or hospice & palliative medicine is highly desirable. Ability to perform comprehensive patient assessments. Excellent communication skills for patient & caregiver education, and interdisciplinary collaboration.
